Our Mission

We deliver old-style Family Medicine in the new healthcare environment. While we use the latest technology, we do not allow the computer to come between you and the doctor.

We believe that preventative care can prolong and provide a higher quality of life for the individual.

We manage chronic diseases and care for acute illnesses. We work hard in the outpatient setting in an effort to keep our patients out of the hospital.

When a higher level of care is needed, we continue to follow and manage our patients in the hospital. In the process, we provide a continuum of care that keeps the patient and doctor connected.

While fewer physicians follow their patients in the hospital, we believe this is the best kind of care to deliver, and our patients agree.

Jon J. Floriano, MD.

Dr. Floriano graduated from Case Western Reserve University School of Medicine in Cleveland and completed his residency in Family Medicine at Metro Health Medical Center. He has been caring for patients in the Middlefield area since 2001 in Family Medicine, Urgent Care, and hospital medicine at UH Geauga Medical Center.
